In the wake of the Columbia shuttle disaster, CMP has dug up a review of a book from a couple of years ago looking at a bunch of large technology-based disasters to look at the various chains of events that caused them (including the Challenger shuttle disaster). The book, Inviting Disaster: Lessons from the Edge of Technology , certainly sounds interesting. I'm always nervous that books like this will fall into the trap of blaming the technology, but it sounds as this book simply focuses on what happened - such that people can learn from the mistakes in the past, and do their best not to repeat them.
